Diabetes Consultant or a DSN see patients jointly with a GP and a Practice Nurse
- Each practice, who has signed up to the GP Local Enhanced Service, has been offered 3 dates for Joint Practice-based Clinics in a year which include the GP and Consultant, with the Practice Nurse attending if possible, to see patients most benefitting from joint review
- The optional extra clinic offered should include the Practice Nurse and Diabetic Specialist Nurse with the GP attending if possible
- Patients are selected by the practice (occasionally patients seen in urgent clinic are reviewed in joint clinic)
- 40min slots are allocated to see patients face to face or 15 min – for virtual review
- Several GPs may attend and bring their own patient cases for discussion
- The Consultant should not see patients in the joint clinic without the presence of the GP
- The DSN should not see patients in the joint clinic without the presence of the PN
Clinics include:
- Seeing patients (eg persistently poor diabetes control, those not engaging with hospital diabetes clinic, different co-morbidities, insulin and GLP-1 starts etc)
- Patients who are strong candidates for a hospital referral
- Discussing some cases without patients being there eg next step in therapy
- Discussing suitable patients that might be discharged from hospital clinics
- Anything the GPs / PNs want to discuss.
