Swindon Community Diabetes Service (SCDS)

The Swindon Community Diabetes Service (SCDS) aims to:

  • Provide support and advice to professionals in GP surgeries, care, nursing and residential homes and to people with diabeteS
  • Improve the knowledge and skills of health care professionals to manage diabetes care through education and training
    Promote personalized care planning through detailed assessment of patients with diabetes
  • Provide accessible services as close to patients’ home or work as possible
  • Promote patients to self care and have a better understanding how to manage their diabetes through a range of education sessions, motivational interviewing, consultation and literature / documentary resources
  • Promote partnership working and a shared care approach between providers to enable patients to receive appropriate care, seamlessly, and in a timely manner
