On Track with Type 2 Diabetes

About this course

Improving your knowledge of diabetes puts you in control and could transform your life. A diabetes course is a key part of your treatment and is crucial to the care you should receive.

On Track with Type 2 Diabetes sessions are two hours long and are run at various locations across Swindon.

We encourage all people to attend an initial course, which includes all the basic information on type 2 diabetes, followed by a second course focusing on food and lifestyle changes.

How to book

Patients can book onto the course by calling 01793 696622 or by sending an email to swiccg.communitydiabetesservice@nhs.net.

Referrals can also be made by GPs and practice nurses.


Living Well with Type 2 Diabetes

About this course

This group course of four sessions (One session per week) teaches you to understand thoughts, feelings and behaviours—this will help you to think about things differently and do things differently. There will be some group discussion, it is up to you how much you get involved.

Living well with Long Term Conditions

About this course

Do you have Cardiac issues, Chronic Obstructive Pulmonary Disease or Type two Diabetes and would like to learn some tools and techniques to help improve your mood?  Then you may be interested in   Lift Psychology’s  free   online program.  It can be accessed from home and is supported by your own trained practitioner . This is based on a psychological approach called Cognitive Behavioural Theory (CBT), which has been shown to be helpful for many people with long term health conditions.

Topics covered include:

  • Understanding how your health impacts on your mood
  • The process of coping with a change in your physical health
  • Techniques for managing difficult thoughts
  • Communication skills
  • Ways to maximise your wellbeing

LIFT Your Voice – Singing and Wellbeing Group

Research shows that taking part in a singing group can have beneficial effects on health and wellbeing. You don’t need to be able to sing, just come along and give it a go in an informal and relaxed environment. The group runs for 4 weekly sessions and is led by an experienced Singing Tutor and member of the LIFT Psychology team.

Onlines resources

Type 2 Diabetes and Me Training Course (Diabetes UK) A free e-learning programme consisting of five modules designed  to help  understand and manage diabetes successfully. You can complete it at your own pace and don’t have to do it all in one go. You will learn about diet, treatment, complications and where to get support.

Lift Psychology is offering a free online programme that can be accessed from home and is supported by your own trained practitioner. This is based on a psychological approach called Cognitive Behavioural Theory (CBT) which has been shown to be helpful for many people with diabetes.

Carbs and Cals For those who are wishing to carbohydrate count, the carbs and cals website will show you thousands of food photos with the carb values clearly shown for each image. The website also offers information on weight loss, the 5:2 diet and healthy eating.
